Descriptions

Ref : TPDHCX/SWC/SW

Responsibilities :

To handle administration tasks related to social work operations
To plan, coordinate and conduct health promotion and education activities, included but not limited to talks, classes, group activities and large-scale events
To liaise with internal and external community partners for primary health care resource hub
To develop and implement of medical social collaboration and community engagement under the DHCE scheme
To carry out other ad-hoc duties initiated by supervisor that deem necessary for project operation, development and fall within DHC scheme service scope

Requirements :

Recognised degree in Social Work
Registered with Social Workers Registration Board
Substantial post-qualification experience practice in social or healthcare service
Experience in community health service and familiar with community resources and health services
Excellent communication skills, self-motivated, flexible, passionate in community engagement
Candidate with less experience will be considered as Social Worker
Require to perform shift duties, including weekends and public holidays, as required by operational needs
