Social Work Coordinator / Social Worker
To cope with the organization's service growth, we are seeking a high caliber individual for the above position.
Descriptions
Ref : TPDHCX/SWC/SW
Responsibilities :
- To handle administration tasks related to social work operations
- To plan, coordinate and conduct health promotion and education activities, included but not limited to talks, classes, group activities and large-scale events
- To liaise with internal and external community partners for primary health care resource hub
- To develop and implement of medical social collaboration and community engagement under the DHCE scheme
- To carry out other ad-hoc duties initiated by supervisor that deem necessary for project operation, development and fall within DHC scheme service scope
Requirements :
- Recognised degree in Social Work
- Registered with Social Workers Registration Board
- Substantial post-qualification experience practice in social or healthcare service
- Experience in community health service and familiar with community resources and health services
- Excellent communication skills, self-motivated, flexible, passionate in community engagement
- Candidate with less experience will be considered as Social Worker
- Require to perform shift duties, including weekends and public holidays, as required by operational needs
