• Degree in Social Work or above, RSW
  
• English Language (Syllabus B) and Chinese Language at Grade E or above in HKCEE or English and Chinese at level 2 or above in HKDSE or equivalent
  
• At least two years post degree social work experiences are preferable
  
• Good command of written English and Chinese
  
• Active, independent, mature, self-motivated, flexible, passionate in community engagement and a good team player
  
• Coordinate and work with different stakeholders and multi-disciplinary team to deliver primary health care service
  
• Implement primary health care service, enhance and facilitate the use of community health resources to the residents of Sham Shui Po
  
• Responsible for the planning and implementation of outreaching service to meet the health need of resident
  
• Shift duties on weekdays and weekends may be required
  
• Working Place : Sham Shui Po
