Requirements :

• English Language (Syllabus B) and Chinese Language at Grade E or above in HKCEE / Level 2 in HKDSE;
  
• Certificate of Registration (General) issued by the Nursing Council of Hong Kong;
  
• Valid Practising Certificate issued by the Nursing Council of Hong Kong;
  
• 3 years' relevant post-qualification experience;
  
• Relevant experience in elderly care and management preferred.

Job Duties :

• Oversee the overall management and operation of the Home Support Team of the Integrated Discharge Support Programme for the Elderly Patients (IDSP) and demonstrate professional leadership and act as a resource for all team members;
  
• Lead the coordinated support of hospital discharges to home and work specifically with the discharge coordinator in partnership and facilitating simple / complex discharges;
  
• Develop effective links and relationships with relevant professional partners, acting as their principal point of contact, to ensure that all discharge arrangements are agreed and information flows effectively, providing as seamless a service as possible for patients and their families;
  
• Understanding of cost effectiveness budgets and detailed understanding of discharge planning processes within an acute setting, including community care, continuing health care and mental capacity issues;
  
• Comply with organisational policies and procedures and requirements of sub-contracted services;
  
• Conduct staff training programmes.

Working Hours :
44 hours per week and on shift when necessary.

Working Location : Kowloon
