• Act as case manager to coordinate health assessments, disease and complication screenings, conduct nurse-led clinic and implement patient empowerment programs;
  
• Plan and deliver health promotion & disease prevention programs;
  
• Coordinate care and facilitate bi-directional referrals across multidisciplinary teams within the District Health Center and community network service providers;
  
• Carry out primary nursing service initiatives in both the DHC Core Centre and its Satellite Centres.

Entry Requirements :

• Possess a Bachelor's Degree in Nursing or above;
  
• Holder of valid Registered Nurse practicing certificate issued by the Nursing Council of Hong Kong;
  
• For Care Coordinator Role : minimum of 5 years' clinical experience as a Registered Nurse;
  
• Post-registration training in one or more of the following areas is an advantage : Diabetes Mellitus Care, Cardiology, Primary Health Care, Community Nursing, Orthopedic Care, Gerontology, or Gynecology;
  
• Demonstrate a passion for community primary healthcare services and a strong sense of professional commitment;
  
• Proficient in managing multidisciplinary teams, with proven strengths in interpersonal communication, coordination and organizational skills.

All candidates of the above posts should possess the following abilities :

• Strong command of both written and spoken English and Chinese;
  
• Proficiency in PC applications such as Microsoft Word, Excel, and Teams;
  
• Self-motivated, detail-oriented, responsible, and trustworthy, with strong interpersonal skills;
  
• Effective communication and organizational abilities.
