• To plan, coordinate and conduct health promotion and education activities, included but not limited to talks, classes, group activities and community based promotion campaign;
  
• Provide case assessment, training, counselling service and care-giver support;
  
• Develop community resources hub and facilitate bi-directional referral with social and health service units;
  
• Liaison with community stakeholders and facilitate Medical Social Collaboration and community engagement.

Entry Requirements :

• Recognized degree in Social Work;
  
• Registered with Social Workers Registration Board;
  
• For Social Worker : At least 5 years of relevant experience, preferable in community healthcare service and familiar with community health resources;
  
• For Senior Social Worker : At least 8 years of relevant experience, with at least 2 years in a supervisory capacity.

All candidates of the above post should possess the following abilities :

• Strong command of both written and spoken English and Chinese;
  
• Proficiency in PC applications such as Microsoft Word, Excel, and Teams;
  
• Self-motivated, detail-oriented, responsible, and trustworthy, with strong interpersonal skills;
  
• Effective communication and organizational abilities.
