Experience working with children, youth and families facing poverty, adversity, safeguarding concerns or multiple needs
Strong knowledge of positive youth development, family-centred practice, risk assessment and two-generation intervention approaches is an advantage
Passionate about improving outcomes for children, youth and families
Conduct screening, risk assessment, triage, and provide tiered interventions for children, youth, and families facing adversity
Develop, implement, and review Individualised Development Plans (IDPs) with children, youth, and families to achieve developmental goals and positive life outcomes
Deliver two-generation interventions that strengthen child development, parenting capacity, caregiver well-being, and family resilience
Mobilise community resources, partnerships, and social capital to connect families with developmental opportunities, support networks, and innovative services
Coordinate multidisciplinary and cross-sector collaboration with service units, schools, social service agencies, and community stakeholders to provide integrated support
Maintain accurate case records, monitor outcomes, support evaluation activities, and utilise digital platforms for case management and service delivery
Contribute to continuous learning, evidence-based practice development, and quality improvement through professional training, research, and knowledge-sharing initiatives
