  • RSW with Degree in Social Work or above
  • Five years post degree social work experience and at least three years working experience in mental health services
  • Implement the JoyAge stepped-care model in DHC / ICCMW / DECC, including delivering intensive case work and therapeutic group intervention
  • Complete all required assessments with clients and ensure their cases have been logged in the IT platform in a timely manner
  • Coordinate with the General Practitioners / Family Doctors to offer pharmaceutic intervention for clients, assist on the administrative work
  • Manage a team of Peer Supporters to assist in the implementation of the project and provide ongoing clinical supervision
  • Participate in regular JoyAge team meeting organized by the HKU team or HKJC, and assist in the implementation of infusion model in district
  • Working Place : Sham Shui Po
