社工 (P3-SW3-IDSP)

Descriptions
- 持政府認可之社會工作副學士 / 高級文憑 / 文憑證書，並符合香港中學會考中文及英文科 (課程乙) 合格，或香港中學文憑考試中文及英文科達 2 級或以上成績；註冊社工；具長者服務相關工作經驗者及「安老服務統一評估機制評估員」資格者優先考慮；
- 為服務使用者及其照顧者提供個案管理服務；
- 提供個別輔導、轉介及跟進相關社區及專業支援服務；
- 安排照顧者支援服務及相關培訓；
- 統籌及籌辦職員培訓活動；
- 建立社區支援網絡，發掘及善用社區資源；
- 督導家居照顧員 (Home Care Workers)，確保服務質素；
- 參與定期個案會議，並與醫院出院策劃小組、註冊護士、物理治療師及職業治療師緊密協作；
- 協助單位運作及行政工作；
- 執行上司指派的臨時任務；
- 此為非整筆撥款津助職位；
- 每週平均工作 44 小時，須輪班。
