Descriptions

To proactively identify and engage hidden elderly persons and vulnerable carers through outreach, home visits and follow-up on referrals.
To provide assessment, counselling, casework and timely intervention for elderly persons, carers and their families.
To plan and deliver groups, programmes and carer support services to enhance service users' wellbeing and strengthen their support networks.
To coordinate community support and service referrals in collaboration with relevant service units, Care Teams and community partners.
To mobilize volunteers and community resources to support elderly persons and carers.
Experience in elderly services, centre management, project management, or administrative supervision is preferred.
Qualification as an accredited assessor under the interRAI-HC 9.3 is an advantage.
Degree in social work.
Work Location : Kowloon City
