• Health Risk Assessment Support : Assist nursing staff in conducting comprehensive health risk assessments, including checking physical conditions and assessing functional abilities
  
• Implementation of Care Plans : Facilitate group classes and workshops aimed at promoting health and wellness among elderly clients as part of individualized care plans
  
• Appointment Arrangements : Schedule and coordinate client appointments for assessments, follow-ups, and other healthcare services
  
• Outreach Booth Management : Organize and manage outreach booths for community health events, providing information and resources to elderly clients and their families
  
• Monitoring and Reporting : Regularly monitor clients' health status and report any changes or concerns to the nursing team for further evaluation
  
• Any other duties assigned by supervisor

Entry Requirements :

• Secondary education is required
  
• Health Care Assistant or Care-related Support Worker Certificate is preferred
  
• Fluent in Cantonese; conversant in English
  
• Able to read and write Chinese and possess knowledge of English
  
• Previous experience in a healthcare setting, particularly with elderly clients, is an asset
  
• Shift duty is required

All candidates of the above vacancies should possess the following abilities :

• Strong command of both written and spoken English and Chinese
  
• Proficiency in PC applications such as Microsoft Word, Excel, and Teams
  
• Self-motivated, detail-oriented, responsible, and trustworthy, with strong interpersonal skills
  
• Effective communication and organizational abilities
  
• Complete any other duties assigned by supervisor(s)
