• Lead and supervise health risk assessments for elderly clients
  
• Mentor junior nursing staff and serve as a clinical resource
  
• Develop individualized care plans and monitor client progress with multidisciplinary approach
  
• Perform advanced assessments for elderly clients with multiple conditions
  
• Identify urgent health risks and prioritize follow-ups
  
• Design health promotion programs and educational materials
  
• Participate in service planning and quality improvement initiatives
  
• Maintain accurate documentation and ensure compliance with clinical guidelines

Entry Requirements :

• Holder of valid Registered Nurse practicing certificate issued by the Nursing Council of Hong Kong
  
• Degree in Nursing or equivalent
  
• Minimum 5 years of working experience in related nursing practice as Registered Nurse
  
• Recognised Post-registration training in
  - Post-registration certificate course in gerontology; or
  - Fellow member of Hong Kong College of Gerontology Nursing; or
  - Recognition under voluntary scheme on advanced / specialized nursing practice in gerontology; or
  - Recognized postgraduate certificate / diploma in gerontology nursing
  
• Experience in care coordination or case management roles
  
• Leadership or supervisory experience
  
• Experience in health education and program development
  
• Track record of quality improvement initiatives

All candidates of the above vacancies should possess the following abilities :

• Strong command of both written and spoken English and Chinese
  
• Proficiency in PC applications such as Microsoft Word, Excel, and Teams
  
• Self-motivated, detail-oriented, responsible, and trustworthy, with strong interpersonal skills
  
• Effective communication and organizational abilities
  
• Complete any other duties assigned by supervisor(s)
