Youth Diabetes Action is a non-profit charitable organization established in 2001, dedicated to supporting children and adolescents with diabetes and their families. We now invite applications for the above position.
Descriptions
Responsibilities :
Counselling Service Leadership: Develop and manage the comprehensive counselling service plan, encompassing new case connections, proactive patient outreach, and hotline management to ensure the delivery of timely support.
Psychosocial Support: Provide professional counselling to individuals with diabetes and their caregivers, focusing on diabetes management and mental well-being, whilst performing regular follow-up assessments.
Programme Design & Management: Design, lead, and promote a diverse range of member initiatives, including the mentorship programme and targeted activities for people with diabetes and their caregivers.
Stakeholder Liaison & Outreach: Serve as a key point of contact between hospitals, medical professionals, and patient groups to coordinate holistic care and advocate for the diabetes community.
Accountability & Reporting: Ensure high standards of quality across all services, achieve deliverables for funded programmes through effective budget and resource control, and compile detailed progress and financial reports for management and donors.
Requirements :
Bachelor's degree in Social Work or above. Must be a Registered Social Worker (RSW) in Hong Kong.
Minimum 5 years of relevant experience, ideally in the NGO or social service sector, with at least 2 years in a supervisory or management role.
Excellent interpersonal and communication skills, with proficiency in written and spoken English and Chinese; fluency in Cantonese is essential.
Proficiency in MS Office (Word, Excel, PowerPoint) and Chinese word processing.
Candidates with less experience may also be considered as Social Worker.
