Social Worker III / Vocational Rehabilitation Counselor

Key Responsibilities :
- Evaluating the vocational abilities, competencies, interests, experience, and educational background of persons with disabilities or chronic illnesses
- Formulating Individual Plans for Employment (IPEs) in communication and collaboration with multidisciplinary rehabilitation professionals in a case-management approach
- Implement the IPEs by engaging with potential employers and relevant stakeholders to facilitate equitable access to employment opportunities
- Monitoring, documenting, and reporting case progress to ensure service quality and timely intervention
- Plan and implement programmes that enhance service users' knowledge, work readiness, and practical skills to support their successful transition into the open job market
Requirements :
- Social Worker III: Registered Social Worker, Diploma in Social Work or above
- Vocational Rehabilitation Counselor: Bachelor Degree in Psychology, Counselling or other related Degree
- 2 years of relevant working experience serving persons with chronic illness or disabilities preferred
- Knowledge in vocational counseling, career and life planning and interpersonal skills training
- Networking skills in building a network with stakeholders
- Experience in vocational training / Supported Employment Training (SET) / On-the-job Training Programme for persons with disabilities is preferable
- Mature, self-initiative, good in communication, analytical and time management skills and multi-disciplinary teamwork
- Good command of both Chinese and English
The successful candidate will normally be appointed on contract terms. Renewal of contract will be subject to the service / operational needs of the department and the candidate's performance.
Work Location :
Tuen Mun (primary), with occasional work in Lam Tin.
