Requirements :

• Either have a University Degree in social work, or have Higher Diploma / Diploma / Associate Degree in Social Work with at least 5 years of experience as Social Worker; registered under Social Workers Registration Ordinance;
  
• English language (Syllabus B) and Chinese language at Grade E or above in HKCEE / Level 2 in HKDSE;
  
• Possess the Child Protection Online Training (Module 1 & 2) e-certificate;
  
• 2 years of post-qualification experience in community health care as a Social Worker is preferred;
  
• Experience working in cancer patient support is preferred;
  
• Strong ability to establish and maintain positive and effective work relationships with volunteers, clients and partners;
  
• Good interpersonal and communication skills, dynamic, independent and collaborative.

Job Duties :

• To plan, implement and monitor Cancer Survivorship Project including case management, training sessions / workshop, outreach activities for symptom management;
  
• Assess the needs of all registered cases and formulate cases' health intervention;
  
• Provide community resources / interventions to strengthen the knowledge and skills in cancer support in the community;
  
• Deliver community events to the public to promote health literacy;
  
• Develop training materials and guideline;
  
• Receive trainings by HKU;
  
• Co-work with clients and academics in progress and outcome documentation and evaluation.
