RSW, Degree in Social Work or above
Good command of Chinese and English
At least 3 years of social work experience in rehabilitation services
Mature, proactive, with strong interpersonal and networking skills
Flexible to work overtime, weekends, or public holidays as required by service needs
Identify and assess vulnerable carers; develop and maintain center-based database
Handle cases, including risk assessment, welfare review, service coordination, and case review
Collaborate with care teams, ICRCs and community partners to facilitate referrals and support
Promote respite services and carer training
Supervise peer support services to relieve caregiving stress and strengthen community support networks
Compile service data, risk profiles and reports to support monitoring, coordination and development
Identify high-risk families and coordinate referrals to strengthen prevention and emergency response
