Social Worker III (equivalent to SWA) / Psychosocial Well-being Officer Community Rehabilitation Network (CRN)
The Hong Kong Society for Rehabilitation (hereunder as "HKSR") is a pioneer in rehabilitation dedicated to providing holistic and personalized services to empower persons with disabilities, chronic illnesses or challenges of aging to lead fulfilling lives, and to advocate for an inclusive society. We are a non-governmental organization receiving subvention from government and funds from private donors and other sources. We invite people with the heart and passion to join our senior management team, to develop with us strategies for services excellence and expansion to prepare for our Headquarters redevelopment to be completed within a few years.

Key responsibilities:
- Provide case management services (project based) on patients with Neuromuscular diseases (NMD) and carers, community rehabilitation and self-management programs, psycho-social support services, health education activities, and organize patients' self-help groups
- Assist in establishment and operation of core service development scheme for self-help groups
- Assist in providing psychosocial support programs
- Deliver health education activities and community programmes for promoting health awareness, disease prevention as well as the importance of peer support and social inclusion
- Provide innovative service in building capacity for continuous development of self-help organizations
Requirements:
- Registered Social Worker with high diploma or associate degree in Social Work or above or Bachelor degree in Psychology
- 1-2 years of experience in self-help group supporting services or working with people with chronic health conditions is preferred
- Innovative, mature, independent, good interpersonal skills and teamwork
- Able to use information technology and multimedia
- Good command of both Chinese and English
- Home visits and hospital work are required on need basis
- Duties in evenings, weekends and public holiday are required on need basis
The successful candidate will normally be appointed on contract terms. Renewal of contract will be subject to the service/operational needs of the department and the candidate's performance.
