Jockey Club Centre for Positive Ageing (JCCPA) is a non-profit organization dedicated to the care of the elderly with dementia, is looking for a highly motivated person to join its multi-disciplinary team to fill the above post.

Descriptions

A recognized bachelor's degree in social work or above and a registered social worker
At least 5 years post-graduation working experience in the elderly service sector, preferably in dementia care
To be responsible for brain health education and cognitive rehabilitation as well as promoting an advance care plan
To be responsible for supporting service in day care and residential care with a case management approach independently
Capable of providing professional assessments and information related to the "Three Instruments of Peace"
Develop online interactive services with IoT and be able to do outreach services with particular needs
