• Outreach Medical Care : Conduct regular scheduled visits to assigned residential homes to provide clinical consultation and medical treatments;
  
• Clinical Management : Diagnose and treat episodic illnesses, manage chronic conditions, and perform regular health assessments for residents;
  
• Care Plan Formulation : Partner with the home care teams (nurses, health workers) to review health status and establish multidisciplinary care plans, including medication reviews and rehabilitation advice;
  
• Vaccination Program : Supervise and administer seasonal vaccinations (e.g., Seasonal Influenza Vaccination) for residential homes and schools;
  
• Infection Control & Advisory : Provide professional guidance and health talk on infection control measures, proper storage of medications, and the maintenance of medical records within the facilities.

Entry Requirements :

• Professional Qualifications : Valid registration with the Medical Council of Hong Kong and a valid Annual Practising Certificate. Fellowship of the Hong Kong Academy of Medicine in the relevant Family Medicine specialty (or equivalent); recognized on the Specialist Register of the Medical Council of Hong Kong preferred;
  
• Experience : Minimum of 3 years of clinical practice experience. Prior experience in outreach medical services, geriatrics, family medicine, or palliative care is highly preferred;
  
• Core Competencies : Excellent interpersonal and communication skills to build trust with residents, their families, and care home staff;
  
• Language Skills : Fluency in spoken Cantonese is essential for clinical consultations; proficient in written and spoken English and Chinese.
