• Responsible in the planning, implementation, monitoring and evaluation of Elderly Health Service (EHS) to be conducted in KCDHCE and report to supervisor;
  
• To communicate with different stakeholders and to establish smooth workflow logistics;
  
• Provide EHS related services, included but not limited to :
  - To act as case manager for EHS clients with referral to appropriate resources or services and follow up with cases regularly;
  - To perform EHS specific assessments, screening etc.;
  - To perform EHS related interventions, e.g. blood taking, health promotion, health education, health coaching etc.;
  
• To plan and support in EHS related services promotion;
  
• To provide training for staff regarding elderly health related topics;
  
• Support in KCDHCE's daily operation;
  
• Any tasks as assigned by supervisor.

Entry Requirements :

• Registered with HK Nursing Council with valid practicing certificate;
  
• Completed the Post-Registration Certificate Course in Gerontology; or As a Fellow member of The Hong Kong College of Gerontology Nursing; or Recognized under the Voluntary Scheme on Advanced and Specialized Nursing Practice in Gerontology Nursing; or Hold a recognized postgraduate certificate or diploma in Gerontology Nursing;
  
• 5 years or above working experience, with experience in elderly related services, is preferable;
  
• Holder of Certificate in Phlebotomy with related experiences is preferable;
  
• Able to work independently, with good organizing and communication skills.
