Descriptions

Responsibilities :

• Assist in counselling service plans, new case connections and proactive patient outreach
  
• Design member initiatives, such as mentorship programmes
  
• Liaise between hospitals, medical professionals, and patient groups
  
• Compile data for progress reports for management and donors

Requirements :

• Registered Social Worker (RSW) with a Bachelor's degree in Social Work or above
  
• Minimum 1 year of experience, ideally in the social service sector
  
• Proficiency in written and spoken English and Chinese; fluency in Cantonese is essential
  
• Candidates with more experience may also be considered as Senior Social Worker

This is a 2-year renewable contract role, with the potential for conversion to a permanent position.
