工作地點：東九龍、將軍澳及西貢；無須輪夜班

Ref : HCST/Diet/MP

Responsibilities :

• Provide dietetic services to elderly of the home care service teams, including conducting home visits, direct assessment, carer education as well as coordinating workshops, programs and menu modification
  
• As an active member of the multi-disciplinary team, provide professional inputs in care plan formulating
  
• Provide professional inputs in service planning
  
• Provide education & coaching to the staff

Requirements :

• Bachelor of Science in Dietetics plus internship and on list of Hong Kong Academy of Accredited Dietitian
  
• Possess 2 years or above post-qualification experience in patient care related practice in dietetic service
  
• Possess good communication and presentation skills
  
• Proactive, mature and independent; possess good computer literacy
