• Health Risk Assessment Support : Assist nursing staff in conducting comprehensive health risk assessments, including checking physical conditions and assessing functional abilities.
  
• Implementation of Care Plans : Facilitate group classes and workshops aimed at promoting health and wellness among elderly clients as part of individualized care plans.
  
• Appointment Arrangements : Schedule and coordinate client appointments for assessments, follow-ups, and other healthcare services.
  
• Outreach Booth Management : Organize and manage outreach booths for community health events, providing information and resources to elderly clients and their families.
  
• Monitoring and Reporting : Regularly monitor clients' health status and report any changes or concerns to the nursing team for further evaluation.
  
• Any other duties assigned by supervisor.

Entry Requirements:

• Secondary education is required.
  
• Health Care Assistant or Care-related Support Worker Certificate is preferred.
  
• Fluent in Cantonese; conversant in English.
  
• Able to read and write Chinese and possess knowledge of English.
  
• Previous experience in a healthcare setting, particularly with elderly clients, is an asset.
  
• Shift duty is required.

All candidates of the above post should possess the following abilities :

• Strong command of both written and spoken English and Chinese;
  
• Proficiency in PC applications such as Microsoft Word, Excel, and Teams;
  
• Self-motivated, detail-oriented, responsible, and trustworthy, with strong interpersonal skills;
  
• Effective communication and organizational abilities.
  
• Complete any other duties assigned by supervisor(s).
