• Deliver pharmacist-led services, including drug counseling, medication reviews, education on adherence, and promotion of medication literacy.
  
• Provide smoking cessation counseling and dispense nicotine replacement therapy (NRT).
  
• Support patients and caregivers in medication-related health management.
  
• Facilitate early detection and appropriate referrals for conditions requiring further medical attention.
  
• Collaborate with healthcare professionals on case management and service development initiatives.
  
• Plan and conduct educational and training programs focused on medication management and safety.
  
• Coordinate the service network with community pharmacies participating in the DHC scheme.
  
• Develop and implement various health promotion programs and patient empowerment initiatives.
  
• Provide drug reviews and individual healthcare sessions on drug management.
  
• Work with a team of healthcare professionals on service developments.

Entry Requirements:

• Degree in Pharmaceutical Studies or equivalent qualification;
  
• Registered with the Pharmacy and Poisons Board of HK and holder of a valid practising certificate;
  
• Minimum of 5 years of cumulative experience as a Registered Pharmacist, preferably with a background in patient care practices within public services or NGOs;
  
• Passionate about community primary healthcare services, with a strong sense of commitment;
  
• Skilled in leading multidisciplinary teams, with proven strengths in interpersonal communication, coordination, and organizational management.

All candidates of the above vacancies should possess the following abilities :

• Strong command of both written and spoken English and Chinese;
  
• Proficiency in PC applications such as Microsoft Word, Excel, and Teams;
  
• Self-motivated, detail-oriented, responsible, and trustworthy, with strong interpersonal skills;
  
• Effective communication and organizational abilities;
  
• Complete any other duties assigned by supervisor(s).
