Responsibilities :

• Provide Pharmaceutical services to DHC Express Scheme service users, including direct assessment, education & coaching, program planning & implementation, related documentation, communication & reporting
  
• Conduct primary health promotion, education talk and mass programs
  
• Act as an active member of the multi-disciplinary team and primary healthcare resource hub to engage related service stakeholder and discipline for primary health & chronic disease management

Requirements :

• Registered pharmacist in Hong Kong with a valid Practising Certificate issued by The Pharmacy and Poisons Council of Hong Kong
  
• Possesses 5 years or more of working experience in related patient care related practice in pharmaceutical service
  
• Proactive, mature and independent; possess good computer literacy
  
• Working Location : Tseung Kwan O
