Social Worker II (Manager II) (Jockey Club End-of-Life Community Care Project : Unison)

Major Duties :
- Uphold the project planning and implementation and provide reports to the funder
- Manage, coach and train staff to enhance work performance and efficiency of operations
- Provide intensive support and counselling to persons with disabilities and their carer, and coordinate individual care plan
- Liaise and coordinate with external parties, including community groups and partner organizations, for in-house and outreach programs implementation
- Experience in working with and motivating volunteers
Requirements :
- Registered Social Worker in Hong Kong; Bachelor degree in Social Work and met the language proficiency requirements of Level 2 or above in Chinese Language and English Language in Hong Kong Diploma of Secondary Education Examination or Hong Kong Certificate of Education Examination, or equivalent; 2 years' post-qualification experience
- Knowledge of advance care planning and end-of-life care will be an advantage
- Experience in working with and motivating volunteers
- Passionate to meet the psychosocial needs of clients and families during the late stage of the life
- Mature, independent, good interpersonal and communication skills
Working Hours : 44 hours per week
Working Location : Lai Chi Kok
