Advanced Practitioner (Social Worker)

Descriptions

Responsibilities :
- Build up effective community networks with external stakeholders
- Facilitate promotion of services and recruitment of clients
- Develop Primary Healthcare Resources Hub with other team members
- Coordinate District Health Center Express program schedules together with other teammates
- Supervise social work and program team
- Deliver direct professional practice in services and team coaching / training as appropriate on case management
Requirements :
- Degree holder or above in Social Work
- Registration with the Social Workers Registration Board
- Eight or more aggregated years of practice in social or medical-social field preferably
