Key Responsibilities :

• Implement primary nursing services, include health promotion, health assessment, health screening, individualized health education and coaching, to the community dwelling elderly in CareAge Nurse Stations
  
• Collaborate with multi-disciplinary team to conduct health services

• Holder of valid Registered Nurse (General) practicing certificate issued by the Nursing Council of Hong Kong
  
• Minimum 3 years of aggregate experience as Registered Nurse with geriatric care / internal medicine care / community health care
  
• Post registration experience in primary health care settings would be an advantage
  
• Good communication and interpersonal skills
  
• Mature and independent
  
• Good computer literacy
