Descriptions
Key Responsibilities :
Responsible for identifying psychosocial needs of K&TDHC clients and organize community health programmes
Plan and Coordinate patient empowerment programs and community support programs in Centre as well as outreaching service spots
Conduct workshops, training sessions and group activities to enhance the volunteers' soft skills such as communication, teamwork and problem-solving
Act as case managers of community rehabilitation and provision of counselling services
Ensure the delivery of social work services in accordance with legislation, policies and guidelines
Work with multi-disciplinary professional teams to deliver primary health care services for K&TDHC
Liaise with different stakeholders including government officials, academics, NGOs, network medical practitioners, professional partners and related parties on community and service collaboration
Perform any other duties assigned by direct supervisor
Requirements :
Registered Social Worker with 5 years' post qualification working experience in community healthcare service, elderly care services and volunteers' development
Good connection with NGOs, social enterprises and other stakeholders in social service
Motivated with strategic mindset in service development
With administrative and supervisory experience
Excellent communication, presentation and interpersonal skills
Attractive salary package will be offered to right person. 5 days' work (44 hours per week, shift duty including Saturday).
