Responsibilities
- Provide counselling to support the psychosocial needs of District Health Centre (DHC) clients relating to their health status
- Organize patient empowerment programs to individuals or in groups
- Compile local community resources for clients' reference when needed
- Organize peer groups and volunteer services
- Assist in promotion and publicity activities of the DHC
- May need to take on flexible hours and occasional Saturday or Sunday duties
Requirements
- Registration with the Social Workers Registration Board
- 3 or more aggregate years of practice in social services
- Experience in chronic disease management; handling cases with chronic diseases and conducting patient empowerment programs will be an advantage
- Familiar with community support resources
The appointment will be made on a fixed-term contract basis.
