Senior Social Worker / Social Worker for Children's Palliative Care Service and Patient Care & Community Service
Children's Cancer Foundation aims at developing our services both on cancer and other life-threatening illness. To cope with our service development, we are now inviting passionate applicant to join us in supporting and walking with young patients with medical complex conditions and their families at difficult times.
Descriptions
(Applicants with less experience will be considered as Social Worker)
Duties :
- To provide case management, counselling and psychosocial services, and group work for children and adolescents suffering from cancer or other life-threatening and life-limiting illness, survivors and their families
- To assess, evaluate and support the social, cultural, financial, emotional, and practical needs of palliative care patients and their carers / families across the hospitals and the community
- To identify service gaps and develop new project initiatives to support patients and families with special needs
- To collaborate and develop relationships with a wide range of healthcare workers, schools, agencies and other NGOs to provide a continuum of care services for patients and their families
- To develop diagnosed groups / new projects for patients and parents
- To organise and support patients' and families' activities / programmes
- To provide bereavement care services for families
- To support the development of children's palliative care service
- To co-ordinate education and developmental programmes for patients after treatment and sibling support programmes
Requirements :
- A recognised degree in social work or counselling from a recognised institution in Hong Kong or equivalent, with registration as a Registered Social Worker in Hong Kong or be eligible for registration / membership of relevant professional bodies
- At least 5 years of post-qualification experience in casework and group work with children/adolescents and families
- Experience in end-of-life care / project co-ordination will be an advantage
- Strong passion for helping children and adolescents with life-threatening and life-limiting illnesses
- Proficiency in both written and spoken Cantonese, Putonghua and English
- Good communication, interpersonal and presentation skills. Proficiency in PC skills, familiar with MS PowerPoint
- Need to work on non-office hours for patients' and families' programmes or for urgent support to families
