The Hong Kong Society for Rehabilitation (hereunder as 'HKSR') is a pioneer in rehabilitation dedicated to providing holistic and personalized services to empower persons with disabilities, chronic illnesses or challenges of aging to lead fulfilling lives, and to advocate for an inclusive society. We are a non-governmental organization receiving subvention from government and funds from private donors and other sources. JCECC "Life Rainbow" End-of-Life Community Care Project is funded by The Hong Kong Jockey Club, with the aim to enhance the quality of life of persons with late-stage chronic illness and their families by using a collaborative partnership approach.

Key Responsibilities :
- Provide case management service to support patients and family caregivers including counselling, psych emotional support, spiritual care, facilitation of family communication, holistic ACP facilitation
- Provide caregiver training and bereavement care
- Developing sustainable volunteer network
- Organize end-of-life care public education programs
Requirements :
- Registered Social Worker with degree in Social Work or above
- A minimum of 3 years work experience
- Experience in working with persons with chronic illness, providing end-of-life care or case management are preferable
- Self-initiative, independent, mature, flexible and good interpersonal skills
Work Location : HK Island East
