Key Responsibilities :

• Responsible for providing health assessment, screening, counselling, health education and other nursing services in K&TDHC including Satellite Centres and outreach projects
  
• Handle full spectrum of daily operations including but not limited to manpower management, clients case follow-up, professional and efficient nursing duties and health care services support
  
• Supervise subordinate to provide health promotion activities and patient empowerment programmes in K&TDHC
  
• Liaise with multi-disciplinary professional teams to deliver primary health care services for K&TDHC
  
• Act as case manager of chronic diseases and community rehabilitation programs
  
• Coach frontline team on customer services skills to ensures the high quality of service standard
  
• Ensure the delivery of nursing services in accordance with legislation, policies and guidelines
  
• Perform any other duties assigned by direct supervisor

• Registered with Hong Kong Nursing Council with valid practicing certificate issued by the Nursing Council of Hong Kong
  
• Degree in Nursing or equivalent with minimum 8 years of working experience in related services
  
• Knowledge in community engagement, health coaching, outreach services and health center management
  
• Recognized post registration training in Primary Health Care / DM care / Community Care / Stroke Rehabilitation / Cardiac Rehabilitation / would be an advantage
  
• Good command of written and spoken English and Chinese
