Senior Social Worker / Social Worker (Applicants with less experience will be considered as Social Worker)

Children's Cancer Foundation aims at developing our services both on cancer and other life-threatening illness. As a pioneer in providing high-quality children's palliative care service for over 25 years in Hong Kong, we would like to invite passionate applicants to join our dedicated team to support and walk with child patients with medical complex conditions and their families at difficult times.

Descriptions

Duties :

  • To provide case management, counselling, and group work for families with children and adolescents suffering from cancer or other life-threatening and life-limiting illness
  • To assess, evaluate and support the social, cultural, financial, emotional, and practical needs of palliative care patients and their carers / families across the hospitals and the community
  • To identify service gaps and develop new project initiatives to support patients and families with special needs
  • To collaborate and develop relationships with a wide range of healthcare workers, schools, agencies and other NGOs to provide a continuum of care services for patients and their families
  • To organise and support patients' and families' activities / programmes
  • To provide bereavement care services for families
  • To support the development of children's palliative care service

Requirements :
  • A recognised degree in social work or counselling from a recognised institution in Hong Kong or equivalent, with registration as a Registered Social Worker in Hong Kong or be eligible for registration / membership of relevant professional bodies
  • At least 5 years of post-qualification experience in working with children, adolescents and families. Experience in working with patients in end-of-life care / project co-ordination will be an advantage
  • Strong passion for helping children and adolescents with life-threatening and life-limiting illnesses
  • Proficiency in written and spoken English and Chinese
  • Need to work on non-office hours for patients' and families' programmes or for urgent support to families
