Key Responsibilities :

  • Organize and coordinate health project in the CareAge Health Center
  • Be responsible for the maintenance of quality patient care and supervision of the ancillary medical service providers
  • Plan and implement primary nursing services, include health promotion, health assessment, health screening, individualized health services and coaching, to the community dwelling elderly
  • Deliver outreach nursing services to home-bound elderly, when necessary
  • Collaborate with multi-disciplinary team to conduct health services
  • Liaise with other parties to contribute in center development

Requirements :
  • Holder of valid Registered Nurse (General) practicing certificate issued by the Nursing Council of Hong Kong
  • Minimum 3 years of aggregate experience as Registered Nurse with geriatric / internal medicine care in hospital
  • Minimum 1 year of aggregate experience as Registered Nurse in primary health care settings
  • Good communication and interpersonal skills
  • Mature and independent
  • Good computer literacy
