Key Responsibilities :
Evaluating the vocational abilities, competencies, interests, experience, and education for persons with disability or chronic illness
Formulating Individual Plans for Employment (IPEs) in communication and collaboration with different rehabilitation disciplines in case management approach.
Implement the IPEs by consulting with potential employers and other stakeholders to facilitate access to employment
Monitoring, documenting, and reporting the progress in the case work
Work Location : Tuen Mun (Main), Lam Tin
Job Requirements :
Social Worker 3 : Registered Social Worker, Diploma in Social Work or above
Vocational Rehabilitation Counselor: Bachelor Degree in Psychology, Counselling or other related Degree
2 years relevant working experience serving persons with chronic illness or disabilities preferred
Knowledge in vocational counseling, career and life planning and interpersonal skills training
Networking skill in building network with stakeholders
Experience in vocational training / Supported Employment / Sunnyway on the Job Training Programme is preferable
Mature, self-initiative, good in communication, analytical and time management skills and multi-disciplinary teamwork
Good command of both Chinese and English
